DEAR PARENTS, 
Thank you for registering to participate in Clement Service Day. We are pleased that your children will also be able to participate in service projects here at Saint Clement School. It is necessary, however, that we have the attached forms signed and returned to us prior to April 25. Please fill out and sign the two downloadable forms listed above for each child who will be taking part in the day. You may drop them off at the Parish Office, send them to Saint Clement at 642 W. Deming Pl. attn: Service Day, 60614, or fax them to 773.281.2509 attn: Service Day.

Reminders:

• Check-in on Service Day will begin at 11:00am in the School Gym
• All children should bring a sack lunch
• All children should be picked up before 5:00pm Mass on Service Day
• Children must be checked in before teams are dismissed to their sites and must remain until parents return from sites. They may be signed in and out only be a parent who is participating in Service Day.
